Job summary

NHS Jobs in London is seeking a Therapy Assistant Practitioner to support psychology, occupational therapy and speech and language therapy services. You will work under supervision to assess patients, progress home-based rehabilitation and deliver simple equipment aids, with responsibilities spanning hospital and community settings.

You will provide information to patients and carers, participate in MDT meetings, maintain records and contribute to treatment planning and discharge processes.

Responsibilities

• Undertake clinical and care duties under supervision of a therapist.
• Carry a full caseload of patients under supervision.
• Assess and provide for equipment needs.
• Progress patients in their homes to maximise function and independence.
• Provide information, teaching and instruction to relatives, carers, colleagues and agencies.
• Promote aims of Therapy and continuation of the treatment programme.
• Participate in multidisciplinary / multi-agency team meetings and case conferences.
• Use skills such as persuasion, motivation and negotiation to gain cooperation.
• Maintain accurate clinical records of all interventions.
• Produce patient-related reports for other disciplines or agencies.
• Input to databases as required.
• Refer to the qualified practitioner when necessary to work independently with specified client group.
• Deliver and install simple aids and equipment in patients’ homes and instruct on use.
• Carry out home visits and access visits, with or without a therapist.
• Manage day-to-day workload and prioritise tasks within the pathway.
• Ensure effective exchange of information across the pathway team.
• Provide guidance and teaching to therapy assistants.
• Be proactive in the community with therapy staff to feedback on patients progress.
• Participate in supervision and appraisal with a senior therapist; attend staff meetings and induction of therapy staff.
• Progress patients exercise programme and mobility, including issuing walking aids.
• Ensure equipment is available and accessible; engage with TCES agenda.
• Communicate with ward staff and educate patients/relatives/carers about therapy aims.
• Maintain records; undertake general administrative tasks.
• Drive a lease car to visit patients’ homes; work flexibly between community and hospital.
• Care duties include personal hygiene, continence, medication management, light housework, shopping and meal preparation; weekend/on-call rota.
