Job summary

HCRG Care Group is seeking a Band 4 Therapy Assistant Practitioner to join our Falls Prevention service in the community around Dartford. You will work within a multidisciplinary team to deliver rehabilitation and therapy programmes designed to improve mobility and independence at patients' homes.

You will support falls prevention, undertake delegated assessments and progress tracking, under supervision of registered therapists, while building skills for autonomous practice in community settings.

Qualifications

• Level 3 qualification or equivalent experience in health/therapy.
• Experience in health, therapy, rehabilitation or care.
• Strong communication and organisational skills.

Responsibilities

• Manage delegated caseload within competencies and seek support from registered therapists.
• Undertake delegated assessments and observations with patients with varied needs.
• Deliver rehabilitation and therapy programmes to improve mobility, safety and independence.
• Support patients following falls and those at risk of falling.
• Monitor progress and escalate concerns as needed.
• Build positive relationships with patients, families and carers.
• Collaborate with Physiotherapists, Occupational Therapists and the MDT.
• Contribute to discharge planning and maintain comprehensive patient records.

An exciting opportunity has arisen for a passionate Band 4 Therapy Assistant Practitioner to join our Falls Prevention service, working alongside our wider Rapid Response and Intermediate Care therapy services.

Our service is focused on helping people remain safe and independent within their own homes wherever possible. You’ll play an important role in supporting patients following a fall, or those at risk of falling, by providing practical rehabilitation and therapy interventions that can help improve their mobility, confidence, safety and independence.

Working under the direction of our registered therapists, you’ll undertake delegated assessments, deliver individualized rehabilitation programmes and monitor patients’ progress. You’ll have the opportunity to work independently within your agreed competencies, while being supported by experienced Physiotherapists, Occupational Therapists and the wider multidisciplinary team.

This is a varied and rewarding community role where you can develop your therapy and rehabilitation skills while making a real difference to patients and their families.

Please note: a full UK driving license and access to a car are essential as travelling to patients within the community is a day-to-day requirement of this role.

Main Responsibilities

As a Band 4 Therapy Assistant Practitioner, you will:

• Manage and priorities a delegated caseload, working within agreed competencies and seeking support from registered therapists when required
• Undertake delegated assessments and observations with patients who have a range of physical and functional needs
• Deliver agreed rehabilitation and therapy programmes to support patients with their mobility, function, safety and independence
• Support people following falls, as well as those who may be at increased risk of falling
• Monitor patients’ progress and response to interventions, recognizing changes or concerns and escalating these appropriately
• Build positive relationships with patients, carers and families, encouraging and motivating patients to work towards their rehabilitation goals
• Work collaboratively with Physiotherapists, Occupational Therapists and colleagues across the wider multidisciplinary team
• Contribute to discharge planning by providing updates on patients’ progress, functional needs and any concerns
• Maintain accurate, timely and comprehensive patient records
• Identify and report clinical risks, working within the boundaries of your role and competence
• Attend and contribute to team meetings, case conferences, training and development opportunities
• Contribute ideas to service development, quality improvement and clinical audit
• Work flexibly across community settings, including visiting patients within their own homes

The Ideal Candidate

You’ll be passionate about helping people maintain their independence and will enjoy working directly with patients as part of a supportive multidisciplinary team.

You do not need to be a registered Physiotherapist or Occupational Therapist for this role. We’re looking for someone with relevant experience within health, therapy, rehabilitation or care who is ready to develop their skills further as a Band 4 Therapy Assistant Practitioner.

You will have:

• A relevant Level 3 qualification or equivalent experience within health, therapy, rehabilitation or care, with the ability to develop and work at Level 4
• Relevant experience within a healthcare, therapy, rehabilitation, community or care environment
• Good communication skills and the ability to build positive relationships with patients, families, careers and colleagues
• Good organizational and prioritization skills
• The confidence to work independently on delegated activities while recognizing when to seek advice or elevate concerns
• Good IT and documentation skills
• A flexible, proactive and team-focused approach
• A commitment to developing your knowledge and completing the competencies and training required for the role

Experience supporting people with mobility, falls, frailty, orthopedic or rehabilitation needs would be advantageous, as would previous experience delivering delegated rehabilitation or therapy programmes.

As this is a community-based role, a full UK driving license and access to a vehicle for work purposes are required.
