Job summary

HCRG Care Group is recruiting a compassionate Healthcare Assistant to support patients in Gravesend, North Kent, across a seven-day community rota. You will work with a multidisciplinary team to deliver rehabilitation and personal care in patients’ homes, maintaining records and promoting independence.

Responsibilities

• Support individualised care and rehabilitation programmes at patients' homes.
• Work with Urgent Care Response and Intermediate Care Team to help patients recover and stay in the community.
• Carry out delegated nursing and therapy activities under supervision.
• Support activities promoting mobility, independence and rehabilitation.
• Provide short-term personal care (washing, dressing, hygiene).
• Support safe and timely discharge from hospital to community.
• Observe patients and report changes promptly to clinicians.
• Encourage participation in care and rehabilitation plans.
• Support families with health promotion and independence.
• Maintain accurate patient records.
• Collaborate with multidisciplinary team for coordinated, person-centred care.
• Provide administrative support as needed.
• Maintain knowledge to deliver safe care.

Are you a compassionate Healthcare Assistant who enjoys making a real difference to people in their own homes?

An exciting opportunity has arisen to join our Urgent Care Response and Intermediate Care Team, supporting patients across the community in North Kent.

Our service plays an important role in helping to prevent avoidable hospital admissions, support timely hospital discharge and enable people to recover and maintain their independence at home.

Working alongside Nurses, Therapists and other members of our multidisciplinary team, you’ll provide hand-on care and rehabilitation support to patients with a range of needs. This could include supporting someone with their rehabilitation programme, helping them regain confidence with everyday activities or providing short-term personal care while longer-term support arrangements are put in place.

You’ll work independently on delegated activities while remaining fully supported by experienced clinical colleagues. With access to mobile technology such as tablets, you’ll be able to work effectively while out in the community and maintain accurate and timely patient records.

This is a varied and rewarding opportunity for someone who is passionate about delivering compassionate, person-centred care and helping people achieve the best possible outcomes within their own homes.

The service operates across seven days, so flexibility to work weekends and bank holidays as part of the rota will be required.

A full UK driving licence and access to a car for work purposes are essential for this community-based role.

Main Responsibilities

As a Healthcare Assistant, you will:

• Support the delivery of individualised care and rehabilitation programmes to patients within their own homes
• Work alongside the Urgent Care Response and Intermediate Care Team to help patients recover, regain independence and remain safely within the community
• Carry out delegated nursing and therapy activities under the direction of qualified members of the team
• Support patients with activities that promote mobility, independence and rehabilitation
• Provide short-term personal care where required, including support with washing, dressing and other hygiene needs
• Support the safe and timely discharge of patients from hospital back into the community
• Observe patients and communicate any changes or concerns promptly to the appropriate clinician
• Encourage and motivate patients to participate in their agreed care and rehabilitation plans
• Support patients and their families with health promotion and maintaining independence
• Maintain accurate and timely patient records
• Work closely with colleagues across the multidisciplinary team to provide coordinated, person-centred care
• Provide appropriate administrative and clerical support to the team when required
• Maintain and develop the knowledge and skills required to deliver safe and effective care
• Work flexibly across the community according to patient and service needs

The Ideal Candidate

You’ll be a caring, proactive and reliable individual who is passionate about supporting people to remain independent and receive high-quality care within their own homes.

You’ll be comfortable working directly with patients and families and able to build positive relationships with people from a variety of backgrounds.

You will have:

• Experience within a health, care or community setting, or relevant transferable experience
• A compassionate and person-centred approach to care
• Strong communication and interpersonal skills
• Good organisation and time‑management skills
• The ability to work effectively both independently on delegated tasks and as part of a multidisciplinary team
• The confidence to recognise when a patient’s needs may have changed and elevate concerns appropriately
• A flexible approach to meet the needs of a seven‑day community service
• Confidence using IT systems and a working knowledge of Microsoft Office packages, including Outlook, Word and Excel
• A willingness to learn and develop your clinical and rehabilitation support skills
