Job summary

HCRG Care Group is seeking a Band 4 Therapy Assistant Practitioner to join our Falls Prevention service in the community around Dartford. You will work within a multidisciplinary team to deliver rehabilitation and therapy programmes designed to improve mobility and independence at patients' homes.

You will support falls prevention, undertake delegated assessments and progress tracking, under supervision of registered therapists, while building skills for autonomous practice in community settings.

Qualifications

• Level 3 qualification or equivalent experience in health/therapy.
• Experience in health, therapy, rehabilitation or care.
• Strong communication and organisational skills.

Responsibilities

• Manage delegated caseload within competencies and seek support from registered therapists.
• Undertake delegated assessments and observations with patients with varied needs.
• Deliver rehabilitation and therapy programmes to improve mobility, safety and independence.
• Support patients following falls and those at risk of falling.
• Monitor progress and escalate concerns as needed.
• Build positive relationships with patients, families and carers.
• Collaborate with Physiotherapists, Occupational Therapists and the MDT.
• Contribute to discharge planning and maintain comprehensive patient records.
