Job summary

IBC Healthcare in Mansfield is seeking an experienced Support Worker to provide dedicated one-to-one support to an adult with learning disabilities in their home and community, using a therapeutic, person-centred approach that promotes routine, confidence and genuine independence.

You will work from an individualised support plan and Positive Behaviour Support framework, helping the person build routines, access the local community, and develop decision-making skills while maintaining dignity

Qualifications

• Experience supporting individuals with learning disabilities, autism, or behaviours of concern
• Understanding of Positive Behaviour Support (PBS) and trauma-informed practice
• Strong de-escalation, communication and observational skills
• Consistency and strong professional boundaries; this person benefits from predictability

Responsibilities

• Deliver person-centred, therapeutic support tailored to the individual's needs, preferences and goals
• Promote independence and meaningful choice in all aspects of daily life
• Support with personal care where required, always maintaining dignity and respect
• Support medication administration in line with policy
• Support daily living tasks, cooking, cleaning, budgeting, appointments as opportunities to build skills and confidence
• Actively encourage community access and social inclusion, tailored to the person's interests
• Follow the support plan, risk assessment and positive behaviour support plan consistently
• Recognise early signs of distress and respond using de-escalation techniques, minimising restrictive practice
• Complete accurate, timely documentation and records
• Work collaboratively with family members, healthcare professionals, and the wider multidisciplinary team
