Job summary

IBC Healthcare in Rotherham is seeking an experienced Support Worker to provide dedicated support to an adult with mental health needs, including learning disabilities, in their own home and community. This role uses a personalised plan and a Positive Behaviour Support framework to promote routines, independence and dignity, with safety and trauma-informed care at its core.

Qualifications

• Experience supporting individuals with learning disabilities, autism, or behaviours of concern.
• Understanding of Positive Behaviour Support (PBS) and trauma-informed practice.
• Strong de-escalation, communication and observational skills.
• Calm, patient, non-reactive approach under pressure.
• Consistency and strong professional boundaries; ability to provide predictable care.
• Experience with personal care when required.
• Belief in the person’s right to make their own choices and live independently.

Responsibilities

• Deliver person-centred, therapeutic support tailored to the individual's needs, preferences and goals.
• Promote independence and meaningful choice in all aspects of daily life.
• Support with personal care where required, always maintaining dignity and respect.
• Support medication administration in line with policy.
• Support daily living tasks, cooking, cleaning, budgeting, appointments as opportunities to build skills and confidence.
• Actively encourage community access and social inclusion, tailored to the person’s interests.
• Follow the support plan, risk assessment and PBS plan consistently.
• Recognise early signs of distress and respond using de-escalation techniques.
• Complete accurate, timely documentation and records.
• Work collaboratively with family members, healthcare professionals, and the wider multidisciplinary team.
