Job summary

IBC Healthcare in Rotherham is seeking a dedicated Support Worker to provide person-centred care for an adult with mental health needs and learning disabilities. The role emphasizes PBS-based, compassionate support at the individual's home and in the community, promoting independence and dignity.

You will work from an individualised plan, support daily living, and collaborate with the care team, family, and professionals.

Qualifications

• Experience supporting individuals with learning disabilities, autism, or behaviours of concern
• Understanding of Positive Behaviour Support (PBS) and trauma-informed practice
• Strong de-escalation, communication and observational skills
• A calm, patient, non-reactive approach and able to stay steady under pressure
• Consistency and strong professional boundaries; this person benefits from predictability
• Experience with personal care where required
• A genuine belief in the person’s right to make their own choices and live as independently as possible

Responsibilities

• Deliver person-centred, therapeutic support tailored to the individual’s needs
• Promote independence and meaningful choice in daily life
• Support with personal care while maintaining dignity and respect
• Support medication administration in line with policy
• Assist with daily living tasks to build skills and confidence
• Encourage community access and social inclusion based on interests
• Follow the support plan and risk assessments consistently
• Recognise early signs of distress and use de-escalation techniques
• Complete accurate, timely documentation and records
• Collaborate with family, healthcare professionals and the care team
