Job summary

IBC Healthcare in Sheffield is seeking an experienced Support Worker to provide one-to-one support for an adult with learning disabilities in their home and community. The role focuses on therapeutic, person-centred care to promote routine, confidence and genuine independence.

You will work within an individualised support plan and PBS framework, helping the person build daily routines, access community activities and develop decision-making skills with dignity and choice at the core.

Qualifications

• Experience supporting individuals with learning disabilities, autism, or behaviours of concern.
• Understanding of Positive Behaviour Support (PBS) and trauma-informed practice.
• Strong de-escalation, communication and observational skills.
• Calm, patient and non-reactive approach under pressure.
• Consistency and strong professional boundaries.

Responsibilities

• Deliver person-centred, therapeutic support tailored to the individual's needs, preferences and goals.
• Promote independence and meaningful choice in all aspects of daily life.
• Support with personal care, maintaining dignity and respect.
• Support medication administration in line with policy.
• Assist with daily living tasks and community access to build skills and confidence.
• Follow the support plan, risk assessment and PBS plan consistently.
• Recognise distress signs and respond with de-escalation to minimise restrictive practice.
• Complete accurate, timely documentation and records.
• Collaborate with family, healthcare professionals and the wider care team.
