Job summary

IBC Healthcare in Sheffield is seeking an experienced Support Worker to provide one-to-one support for an adult with learning disabilities in their home and community. The role focuses on therapeutic, person-centred care to promote routine, confidence and independence, with emphasis on dignity and choice.

You'll work within an individualised support plan and PBS framework to help the person build daily skills, access community activities, and navigate anxiety or distress with a calm,

Qualifications

• Understanding of Positive Behaviour Support (PBS) and trauma-informed practice.
• Strong de-escalation, communication and observational skills.
• Calm, patient and non-reactive approach under pressure.
• Consistency and strong professional boundaries; value predictability.
• Experience with personal care where required.

Responsibilities

• Deliver person-centred, therapeutic support tailored to the individual's needs and goals.
• Promote independence and meaningful choice in daily life.
• Support with personal care while maintaining dignity and respect.
• Assist with medication administration in line with policy.
• Support daily living tasks and budgeting, using opportunities to build skills.
• Encourage community access and social inclusion tailored to the person’s interests.
• Follow the support plan, risk assessment and PBS plan consistently.
• Recognise distress and respond using de-escalation techniques to minimise restrictive practices.
• Complete accurate, timely documentation and records.
• Work collaboratively with family members, healthcare professionals, and the wider multidisciplinary team.
