  • Set up partnerships with statutory and VCFS organisations to support people with needs spanning many domains of multiple disadvantage and social exclusion.
  • Set up new multi-agency case review meetings in North Somerset, South Gloucestershire and Bath and North East Somerset, and work closely with the existing multi-agency case review meeting in Bristol.
  • Contribute to the promotion, strategic and operational planning of the service, setting up strategic steering group meetings in each area and driving forwards system change when blocks and/or barriers are encountered.

The role will be based at St Mungo’s offices in central Bristol, with regular travel within Bath and North-East Somerset, Bristol, North Somerset and South Gloucestershire.
