Job summary

Waythrough is seeking a Team Manager for the Hospital to Home Service in Sheffield. You will lead a dedicated team delivering specialist interventions for individuals with alcohol misuse in hospital settings and coordinate safe transitions back to home and community.

• Leadership experience in health or social care settings.
• Ability to manage, coach and supervise diverse staff including volunteers.
• Experience improving service delivery and meeting quality targets.

Responsibilities

• Lead the Hospital to Home Service team and ensure delivery of specialist interventions.
• Support the Service/Area Manager to develop and monitor processes and KPIs.
• Mentor, recruit, induct and appraise team members; manage caseloads and risk assessments.
• Maintain records, reports and feedback to improve service quality.
• Deputise for the Service/Area Manager when needed.
• Ensure health and safety and accessibility of services.
