Job summary

Waythrough is hiring a Team Manager for the Hospital to Home Service in Sheffield. You will lead a dedicated team delivering specialist interventions in acute hospital settings, ensuring safe transitions back to home and community care.

You’ll mentor staff, manage performance, and maintain high-quality assessments and records. You will work within a multidisciplinary team, align with NICE/DOH/NTA standards, and report on KPIs while developing services to improve outcomes for individuals affected

Qualifications

• NVQ Level 3 in Health & Social Care (or above) or equivalent.
• Experience in leading and developing staff with supervision.
• Understanding of evidenced-based interventions (MI, PSIs, CBT, ITEP).
• Experience maintaining confidentiality and professional boundaries.
• Experience completing service user assessments including risk assessments.
• Experience with NICE/DOH/NTA/NMC & Caldicott standards.
• Ability to extract, analyze and report on management information.
• Proficient in Microsoft Outlook and Office programmes.

Responsibilities

• Lead and manage a team delivering hospital-to-home services across Sheffield.
• Support the Service/Area Manager to develop, monitor and improve processes and KPIs.
• Lead quality service delivery elements and resource management.
• Mentor, coach and develop staff, including volunteers, with training and supervision.
• Manage a caseload with high-quality assessments and risk assessments.
• Maintain accurate records and deliver verbal and written reports.
• Analyse data to drive service improvements and participate in management discussions.
