Job summary

Waythrough is hiring a Team Manager for the Hospital to Home Service in Sheffield. You will lead a dedicated team delivering specialist interventions in acute hospital settings, ensuring safe transitions back to home and community care.

You’ll mentor staff, manage performance, and maintain high-quality assessments and records. You will work within a multidisciplinary team, align with NICE/DOH/NTA standards, and report on KPIs while developing services to improve outcomes for individuals affected

Qualifications

  • NVQ Level 3 in Health & Social Care (or above) or equivalent.
  • Experience in leading and developing staff with supervision.
  • Understanding of evidenced-based interventions (MI, PSIs, CBT, ITEP).
  • Experience maintaining confidentiality and professional boundaries.
  • Experience completing service user assessments including risk assessments.
  • Experience with NICE/DOH/NTA/NMC & Caldicott standards.
  • Ability to extract, analyze and report on management information.
  • Proficient in Microsoft Outlook and Office programmes.

Responsibilities

  • Lead and manage a team delivering hospital-to-home services across Sheffield.
  • Support the Service/Area Manager to develop, monitor and improve processes and KPIs.
  • Lead quality service delivery elements and resource management.
  • Mentor, coach and develop staff, including volunteers, with training and supervision.
  • Manage a caseload with high-quality assessments and risk assessments.
  • Maintain accurate records and deliver verbal and written reports.
  • Analyse data to drive service improvements and participate in management discussions.

As Team Manager, you will provide leadership, management, and operational oversight to a dedicated team delivering specialist interventions within acute hospital environments, safe transition back into the home and community alongside supporting into Likewise for specialised and tailored support. **What You'll Do:*** Implement, develop and assist in the management of services delivered by the team within a defined specialist area.* Support the Service/Area Manager to develop, deliver, monitor and improve processes, control systems and work environments to meet quality requirements and contractual needs including KPIs.* Develop, deliver and operationally lead quality service delivery elements, pathways and resources that meet service and local need.* Manage, mentor, coach and ensure the development of staff reporting to you, including volunteers. This includes involvement in delivering appropriate training, recruitment, induction, monitoring performance, managing absence and appraisals/supervisions.* Manage a caseload of service users, completing and reviewing high quality and comprehensive assessments, risk assessments and support plans, including liaising with family members, significant others and professionals / agencies as appropriate.* Maintain accurate and up to date records and reports and provide written and verbal reports as required.* Analyse information and utilise this analysis to continuously improve the service.* Work as part of the local management team and participate in working thematic groups and corporate activities.* Contribute to the delivery of company strategy by communicating and implementing objectives and priorities at project level.* Take responsibility for Health & Safety issues in the working environment.* Actively engage service users in future service design/changes to delivery.* Ensure all services delivered are accessible paying particular attention to underrepresented groups.* Deputise for the Service/ Area Manager in their absence.* Reduce substance related harm to the individual and wider community. **To Succeed in This Role, You’ll Need:**Essential**:*** NVQ Level 3 in Health & Social Care (or above or equivalent).* Experience of leading and developing people, including providing effective and regular supervision.* Understanding of and ability to implement evidenced based interventions, including brief interventions MI, PSIs, CBT and ITEP.* Experience of maintaining confidentiality and working within professional boundaries.* Experience of completing service user assessments including comprehensive risk assessments.* Experience of working with best practice frameworks e.g. NICE, DOH, NTA, NMC, Care Quality Commission & Caldicott Standards.* Ability to extract, analyse and report on management information.* Proficient in Microsoft Outlook Office programmes To view the full job and person specification, please click here. **What We Offer**We value the people who make a difference every day.
