Job Description

We are seeking an experienced Team Leader to join our dedicated team. The role involves developing and coordinating learning disabilities supported housing schemes and domiciliary services across Blackpool.

Responsibilities include:

1. Leading a team of staff, providing supervision, coaching, and performance management.
2. Ensuring the delivery of person-centred support to enable customer independence and wellbeing.
3. Overseeing staff deployment, identifying training needs, and maintaining accurate records.
4. Responding to referrals, conducting initial assessments, and ensuring co-produced support plans.
5. Establishing goals and pathways towards independence, organizing reviews, and involving service users in planning.
6. Collaborating with local multidisciplinary teams and community agencies to provide holistic support.
7. Maintaining high standards of safety and quality, and fostering an inclusive environment.
8. Facilitating engagement, involvement, and peer support among service users.
9. Implementing quality monitoring and performance indicators, and planning improvements.
10. Providing excellent customer care and communicating proactively about risks and welfare.
11. Upholding ethical duties of care and safeguarding vulnerable individuals.
