Job summary

Brook Street is recruiting on behalf of a well-established supported living provider for a Care Manager - Supported Living in Port Talbot. The role combines 16 hours of management with 21 hours of direct support, leading a team of about 12 staff and ensuring high-quality, person-centred care.

Salary £29,347.24 - £30,987.84 per annum, 37 hours per week, permanent contract. Applicants should have 2+ years in health & social care management, QCF/NVQ Level 3, driving licence, and strong leadership

Qualifications

• Minimum 2 years' management experience within health and social care.
• QCF/NVQ Level 3 in Health & Social Care or above.
• Experience supporting individuals with challenging behaviours.
• Full UK driving licence.
• Excellent leadership, communication and organisational skills.
• Flexible, hands-on and positive approach.

Responsibilities

• Lead and develop a team of approximately 12 support staff.
• Conduct supervisions, appraisals and staff performance reviews.
• Manage rotas and ensure safe staffing levels.
• Oversee care plans, risk assessments and support documentation.
• Promote person-centred practices and positive behaviour support approaches.
• Ensure compliance with organisational policies and regulatory standards.
• Build positive relationships with families, professionals and external agencies.
• Provide direct support as required and lead by example.
• Participate in the organisation's response/on-call service.
