Job summary

Concept 11 is seeking a dedicated care worker to provide person-centred support for individuals with complex needs, including learning disabilities and autism. You will assist with daily activities and personal care, following individual plans and risk assessments while promoting dignity, independence, and safety.

The role requires experience in health and social care, with a focus on positive behaviour support and de-escalation.

Qualifications

• Previous experience in health and social care, supporting individuals with complex needs.
• Experience supporting people with learning disabilities, autism, mental health needs or behaviours that challenge.
• Willingness to undertake mandatory training and DBS checks as required.

Job description

Provide person-centred care and support to individuals with complex needs, including learning disabilities, autism and behaviours that may challenge. Duties include assisting with personal care and daily activities, following individual care plans and risk assessments, supporting community access and meaningful activities, recognising behavioural triggers, and using appropriate de-escalation techniques. Maintain accurate records, promote dignity and independence, and work closely with colleagues to ensure individuals' safety and wellbeing.

Previous experience in health and social care, supporting individuals with complex needs.Experience supporting people with learning disabilities, autism, mental health needs or behaviours that challenge.Practical experience using positive behaviour support and de-escalation techniques.Understanding of safeguarding, duty of care, confidentiality and person-centred care.Ability to remain calm and professional in challenging situations.Good communication, teamwork and record-keeping skills.Ability to follow individual care plans, risk assessments and behavioural support plans.Flexibility to work shifts and availability for 38–48 hours per week.Unrestricted Right to Work in the UK. Visa sponsorship is not available.Willingness to undertake mandatory training and DBS checks as required.
