Job summary

Glassmoon Services Ltd is recruiting for a Support Worker to provide 24-hour support to individuals with autism and/or learning disabilities. You will deliver person-centred care, follow plans, and help them achieve daily goals and community integration.

Qualifications

• Experience in social care or support roles is a plus.
• Ability to follow person-centred care plans and safeguarding policies.
• Excellent interpersonal skills and communication with individuals and teams.

Responsibilities

• Deliver person-centred care to individuals with autism and/or learning disabilities.
• Maintain and update Support Plans and essential information for care delivery.
• Help individuals build and maintain relationships with family, friends and community.
• Collaborate with the Team Leader/Senior Support Worker to monitor needs and wellbeing.
