Job summary

Cygnet Oaks in South Yorkshire is seeking a confident Care & Support Worker to join our Level 2 rehab team supporting men with mental health needs. You’ll work 42 hours a week across days, nights and weekends, providing physical and emotional support in a 35-bed inpatient setting.

You’ll be part of a multi-disciplinary MDT, with ward-based pathways (Lodge and House), focused on enabling service users to progress toward independent living.

We are looking for a confident Care & Support Worker with a passion for delivering outstanding care.

You’ll be working 42 hours a week (Days Nights and Weekends) in a Level 2 Rehab setting working with male mental health client group at Cygnet Oaks.

Cygnet Oaks is a 35 bed service specialising in high support inpatient rehabilitation (level 2) for men, providing a multi-disciplinary treatment programme designed to assist individuals with their return to independent living.

Depending on the individual’s level of need, at Cygnet Oaks we provide different levels of support in 2 wards, the Lodge and the House;

• Lodge– this is a 20 bed admission ward where most service users will start their recovery journey at Cygnet Oaks. Individuals who are admitted to this ward are generally stepping down from an acute or a low secure service
• House– a 15 bed service for individuals to continue along their care pathway. The Lodge also contains a 4 bed step-through service. This is situated in its own corridor and has a separate living space to prepare and develop individuals towards a community placement whilst still having input from the multi-disciplinary team (MDT).
