Job summary

Active Care Group in Northampton are recruiting a Support Worker at Christchurch View to help people in a rehabilitation pathway from post-acute care through to community-based transitional rehab. The role is full-time nights, 37.63 hours per week, with a salary of £25,292 per year (FTE, pro rata).

• Collaborate with the clinical team to ensure quality care.

Christchurch view - At Christchurch View, we offer a rehabilitation pathway from post-acute Rehabilitation through to specialist community-based transitional rehabilitation. What you'll be working:

• Working 37.63hrs a week
• Annual salary £25,292 (FTE) Per annum (Pro rata)
• Follow 2 weeks rota
• Full time Nights

What you'll be doing:

• Build relationships with the people we support, which are open and two way
• Support to undertake their rehabilitation programme
• Establish methods of communication with the people we support, reducing barriers when necessary
• Support with activities of daily living and personal care when needed
• Communicate any issues to the clinical team
• Accurately report and/or record work activities
