Job summary

Portsmouth City Council is seeking a Support Advisor for the new Support Hub to help children, young people and families access timely support, guided by a relational, strengths-based and preventative approach.

You will work in a multidisciplinary team, providing advice, guidance and practical support, signposting to services, and coordinating Family Support Plans with schools, Family Help, MASH and partners to ensure effective early intervention.

Qualifications

• Experience with safeguarding practice and early help
• Ability to coordinate services across agencies
• Strong communication and relationship-building skills

Responsibilities

• Provide advice, guidance and signposting to families and professionals
• Coordinate and monitor referrals and workflows within the Support Hub
• Collaborate with schools, Family Help, MASH and partner agencies
• Develop and coordinate Family Support Plans
• Support transition of families to wider community services
• Use data to inform decisions and record outcomes
• Attend locality network meetings to build partnerships
