Job summary

Home Group is looking for a caring, proactive professional to help people with complex needs live more independently. You’ll lead support planning, coordinate our Support Workers and deliver person-centred care, while working with multi-agency teams to safeguard and empower customers.

You’ll work 8am–8pm with every third weekend, a service-based role, enhanced DBS paid, and benefits including 34–39 days leave, a health cash plan and pension matching helping families.

Qualifications

• Experience in supporting others, ideally in a supported housing setting.
• Experience assessing referrals and coordinating support.
• Ability to work on own initiative, stay calm under pressure and be resilient.

Responsibilities

• Lead support planning and coordinate our Support Workers to deliver person-centred support.
• Balance support, safeguarding, admin and meetings while guiding colleagues and managing caseloads.
• Support Customers with daily tasks like budgeting, cooking and attending appointments.
• Work with multi-agency teams to safeguard and empower our customers.
• Help customers build independence and prepare for their move-on into the community.
