West Suffolk NHS Foundation Trust's Suffolk Stroke Early Supported Discharge team is hiring for a Band 4 part-time, permanent role delivering individualised stroke rehabilitation plans with patients, families and carers. The post operates across Suffolk with a base in Ipswich and travel between Bury St Edmunds and Ipswich.

Formación

• Foundation degree in healthcare or equivalent.
• NVQ level 3 in a relevant subject with evidence of further study at diploma level after qualifying.
• NVQ level 4 in a relevant subject with evidence of work-based experience in a relevant area.
• GCSE Maths and English at grade C/4 or above.

Responsabilidades

• Deliver individualised stroke rehabilitation care plans with patients, families and carers.
• Work as part of the interdisciplinary team, supported by senior therapists.
• Take part in a structured training programme and receive regular supervision.
• Travel across Suffolk between the Bury St Edmunds and Ipswich offices, with a base at Allington Clinic in Ipswich.
