Job summary

West Suffolk NHS Foundation Trust is recruiting a Band 3 health care support worker to deliver specialist, individualized stroke rehabilitation in patients' homes and the community, within the Suffolk Stroke Early Supported Discharge team.

You will work as part of a multidisciplinary team, travel across Suffolk with a base at Allington Clinic in Ipswich, and participate in regular supervision and a structured training programme.

Qualifications

• GCSEs in English and Maths at grade A–C or equivalent.
• Experience in care or rehabilitation setting.
• Understanding of rehabilitation needs of patients.

Responsibilities

• Deliver specialist, individualised stroke rehabilitation to patients in their homes and the community.
• Work with patients, families and carers to deliver individual rehabilitation care plans.
• Provide specialist therapy input to optimise recovery and independence.
• Collaborate with therapists, nurses and other staff as part of the MDT.
• Travel across Suffolk with a base at Allington Clinic in Ipswich.
• Participate in supervision and a structured training programme with senior therapists.
