Job summary

Active Care Group is recruiting a Staff Nurse (Tracheostomy and Neurorehabilitation) at the Hothfield Brain Injury Rehabilitation Centre. The role involves leading nursing input for residents with brain injuries and neurological conditions, with a focus on outcome-driven care planning.

You will work 37.63 hours per week, on a rota including days, nights and weekends, and support MDT and clinical review processes in a neurorehabilitation setting.

Qualifications

• Current NMC Pin registration (RGN) required.
• Legal right to work in the UK.
• Up-to-date experience/knowledge of tracheostomy care essential.
• Up-to-date experience/knowledge in management of neuro conditions essential.
• Experience working in a neurorehabilitation setting.

Responsibilities

• Act as Primary Nurse, taking overall responsibility for the nursing input residents receive.
• Contribute to MDT working, risk assessment management and clinical improvement programmes.
• Provide written nursing reports for statutory and clinical review processes.
• Follow the NMC Code of Professional Conduct and act as named nurse or key worker.
• Ensure comprehensive assessment for patient pathways and care plans.
• Take charge of a designated clinical area.
• Maintain high-standard clinical documentation and patient notes.
• Provide a highly specialised range of clinical interventions relevant to patients’ needs.
• Work autonomously to provide a safe, specialised service to patients.
