Job summary

Cambridgeshire County Council is seeking a qualified Social Worker to join the Prevention and Early Intervention (PEI) Assessment team on a county-wide basis. You will undertake Care Act assessments, support carers, and help individuals access appropriate care and support early.

The role involves managing a caseload, taking part in duty rotas, and collaborating with Adult Social Care, Health services and Prevention & Early Intervention services to safeguard vulnerable adults and promote timely,

Qualifications

Qualified Social Worker with Social Work England Registration.
Strong knowledge of the Care Act 2014 and Mental Capacity Act 2005.
Experience working in multi-disciplinary teams to achieve outcomes.
Excellent communication, planning and organisational skills.
Ability to work on own initiative and under pressure.

Responsibilities

Completing assessments with adults who may need care and support.
Working with unpaid carers to understand their needs and undertake carers' conversations or assessments.
Developing care and support plans.
Contributing to safeguarding referrals and identifying risks to vulnerable adults.
Applying the Mental Capacity Act principles and conducting capacity assessments.
Signposting to preventative and community services and promoting timely access to support.
Focusing on preventing, reducing and delaying care and support needs.
