Stoke-on-Trent City Council is seeking a Social Worker to join the Learning Disability Team. The role is based at Riverside Employment Training and requires 30 hours per week.

The post is open to both internal and external applicants. You will receive referrals, undertake assessments and reviews, manage safeguarding concerns, and develop care and support plans with multidisciplinary teams, ensuring a needs-led approach and appropriate packages within the Care Act framework.

Qualificações

• Qualified social worker with knowledge of Adult Social Care and safeguarding in community settings.
• Experience of conducting social care assessments and care planning.
• Ability to work in partnership with service users, carers and MDTs.

Responsabilidades

• Social Care assessments for adults and carers.
• Reviews, care and support planning and safeguarding inquiries.
• Maintain accurate records on IT systems and report writing.
• Work within Care Act and Mental Capacity Act frameworks.
