You will join a respected Adult Social Care service, rated ‘Good’ by the Care Quality Commission, collaborating with teams including transition workers and health professionals to empower adults with learning disabilities.

Responsibilities

• Conduct assessments to understand individual needs and develop robust support plans.
• Review support plans to ensure they reflect changing needs.
• Implement safeguarding measures to protect vulnerable adults.
• Fulfill Care Act responsibilities to promote individual independence and well-being.
• Collaborate with carers, external agencies, and multidisciplinary teams to secure positive outcomes.
• Embrace the “Ways to Wellbeing” approach, ensuring every decision centres on the individual’s aspirations and needs.
