Job summary

Qualifications

• Experience in Adult Social Care or equivalent
• Ability to manage safeguarding concerns and work with multi-disciplinary teams

Responsibilities

• Social Care assessments for adults and carers and related reviews
• Care and support planning and Continuing Health Care decision support tools
• Manage safeguarding enquiries and maintain accurate client records
• Coordinate with housing, community services and family support systems
• Advocate for patients’ rights and involve them in decisions
