Job summary

Liquid Personnel Limited. is seeking a Social Worker for its Hospital Discharge Team in Manchester. You will work with patients preparing for discharge, coordinating multi-agency plans and ensuring safeguarding and person-centred care throughout the process.

The role involves completing Care Act assessments, MCA considerations and collaboration with health, social care, and housing partners to support safe and timely discharges.

Qualifications

• Must be Social Work England registered and eligible to work in the UK.
• Hold a full UK driving licence.
• Post-qualification experience in social work desirable.
• Degree level or equivalent in Social Work required.

Responsibilities

• Maintain comprehensive case records and attend discharge planning meetings.

Liquid Personnel is recruiting Social Worker for its client's Hospital Discharge Team at Manchester.

What will your responsibilities be?

• Provide timely, person-centred social work assessments to patients preparing for discharge from hospital.
• Assess patients’ care and support needs and identify appropriate discharge pathways and support arrangements.
• Work collaboratively with patients, families, carers and multidisciplinary teams to plan safe and effective discharges.
• Coordinate discharge planning with health, social care, housing, community and voluntary sector services.
• Complete Care Act assessments and develop appropriate care and support plans where required.
• Undertake Mental Capacity Act assessments and contribute to best-interest decisions where appropriate.
• Identify and respond to safeguarding concerns, ensuring appropriate action is taken to protect adults at risk.
• Support patients and families to understand their options and make informed decisions about care and support.
• Arrange and coordinate packages of care, reablement, rehabilitation, supported accommodation or other appropriate services.
• Contribute to complex discharge planning, including cases involving multiple agencies, family dynamics or challenging circumstances.
• Advocate for patients’ wishes, rights, dignity and independence throughout the discharge process.
• Maintain accurate, timely and comprehensive case records in accordance with statutory and organisational requirements.
• Attend and contribute to multidisciplinary meetings, discharge planning meetings and case discussions.
• Manage a varied caseload effectively, prioritising urgent and high-risk cases.
• Promote strengths-based and preventative approaches to reduce avoidable hospital admissions and support people to remain independent in the community.
• Work in accordance with relevant legislation, professional standards, local policies and safeguarding procedures.
• Develop effective working relationships with hospital staff, community teams, local authorities and external partner agencies.
• Support timely discharge while ensuring that appropriate assessments, safeguards and support arrangements are in place.

Qualifications & Experience:

To be successful in this role you must have,

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK driving licence
• Post-qualified experience
• Degree level or equivalent in Social Work
