Job summary

Liquid Personnel Limited is recruiting an Adult’s Social Worker to join its client’s Mental Health team in Manchester. You will manage a caseload of adults with complex mental health needs, conduct comprehensive assessments and develop personalised care plans with service users and carers.

Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s Mental Health team, based in Manchester

What will your responsibilities be?

• Manage a caseload of adults experiencing a range of complex and enduring mental health difficulties, providing person-centred and strengths-based social work interventions.
• Complete comprehensive assessments of social care, mental health, safeguarding, risk and support needs.
• Develop, implement and review personalised care and support plans in partnership with service users, carers and the wider multidisciplinary team (MDT).
• Undertake Care Act assessments and reviews, including consideration of eligibility, wellbeing, prevention and least restrictive support options.
• Contribute to risk assessments and risk management plans, responding appropriately to changes in presentation, safeguarding concerns and crisis situations.
• Work collaboratively with psychiatrists, nurses, occupational therapists, psychologists, support workers and other professionals to provide coordinated care.
• Support service users to develop and maintain independence, social inclusion, meaningful activity, housing stability and community connections.
• Provide advice, information and advocacy around housing, benefits, employment, finances, social care and other wider determinants of wellbeing.
• Work proactively with families, carers and relevant professionals, ensuring their views and needs are appropriately considered.
• Safeguard adults at risk and take appropriate action in line with local safeguarding procedures and statutory responsibilities.
• Support service users through transitions between services, including hospital discharge, community support and onward referrals.
• Maintain accurate, timely and high-quality case records in accordance with professional, organisational and legal requirements.
• Participate in MDT meetings, clinical reviews, care programme processes and other relevant meetings.
• Work within relevant legislation, policies and professional standards, including the Care Act, Mental Health Act, Mental Capacity Act and safeguarding frameworks.
• Promote service-user choice, autonomy, recovery, equality, diversity and human rights in all aspects of practice.
• Build effective therapeutic and professional relationships, using appropriate communication and social work skills to engage people who may be reluctant or difficult to engage with services.
• Identify and respond to changes in need, escalating concerns and accessing additional support where appropriate.
• Work with partner agencies and community organisations to promote integrated, preventative and recovery-focused support.
• Participate in supervision, reflective practice, training and continuous professional development.
• Contribute to service development, quality improvement and the ongoing development of effective social work practice within the CMHT.

Qualifications and Experience:

To be successful in this role you must have;

• Social Work England registration
• Eligible to work in the UK
