Liquid Personnel is recruiting a Social Worker for its Adults Discharge to Assess Team in Bristol. You will support citizens during transitions from hospital to community settings, ensuring safe and effective discharge outcomes.

Qualifications

• Qualified Social Worker with Social Work England registration.
• Experience within Adults Services, ideally discharge or hospital-based teams.
• Strong knowledge of Care Act, MCA, and safeguarding legislation.
• Ability to work in a fast-paced environment and manage competing priorities.

Responsibilities
