Job summary

Liquid Personnel is seeking a skilled Social Worker to join the Hospital Pathway Adult Social Work Team in Sutton. You will support adults moving from hospital to community services, coordinating timely discharge and safeguarding as needed.

The role requires SW England registration, a UK driving licence, and post-qualification experience. A full UK working entitlement is essential, with a commitment to person-centred discharge planning and close collaboration with healthcare colleagues.

Qualifications

• Degree level or equivalent in Social Work required.

Responsibilities

• Conduct comprehensive assessments and develop discharge plans with healthcare teams and families.
• Manage a caseload with timely hospital discharge to community or residential settings; address safeguarding concerns.
• Provide ongoing advocacy and ensure patient rights and needs are prioritised during discharge.

Job Description:

Liquid Personnel is currently recruiting a skilled and compassionate Social Worker to join the Hospital Pathway Adult Social Work Team. The successful candidate will play a critical role in supporting adults during their transition from hospital care to community-based services, ensuring a smooth and effective discharge process.

What will your responsibilities be?

• Conduct comprehensive assessments of patients' needs and develop individualized discharge plans in collaboration with healthcare professionals, patients, and their families.
• Manage a caseload of patients, ensuring timely and coordinated discharge from the hospital to appropriate community or residential settings. Identifying and addressing any safeguarding concerns to protect vulnerable adults from abuse, neglect, or exploitation
• Provide ongoing support and advocacy for patients, ensuring their rights and needs are prioritized throughout the discharge process.
