Qualifications

Liquid Personnel is recruiting Social Worker for its client's Transfer of Care Team at Peterborough.

This is an excellent opportunity for a dedicated practitioner to support adults with complex care and support needs through effective assessment, care planning, safeguarding interventions, and coordinated discharge and transfer of care arrangements.

The successful candidate will work with a diverse range of adults, including individuals with physical disabilities, sensory impairments, learning disabilities, and long-term conditions. You will play a key role in ensuring care is well coordinated for those most at risk of hospital admission or long-term care placement, whilst promoting independence, choice, and wellbeing through strengths-based and person-centred practice.

This role requires a confident and experienced Social Worker capable of managing a varied caseload, making complex decisions, undertaking safeguarding work, and working collaboratively with service users, carers, health professionals, and partner agencies to achieve positive outcomes.

What will your responsibilities be?

• Managing a varied caseload of adults with complex care and support needs, ensuring timely intervention and effective case management.
• Undertaking Care Act 2014 assessments, reviews, reassessments, and support planning to identify eligible needs and improve outcomes.
• Formulating, implementing, monitoring, and reviewing Care and Support Plans to ensure the most effective use of available resources.
• Promoting independence, choice, control, and personalisation, including the active promotion of Direct Payments and personalised support arrangements.
• Coordinating services and support arrangements to ensure care is effectively delivered and regularly reviewed.
• Undertaking safeguarding enquiries and implementing safeguarding protection plans in accordance with Adult Safeguarding legislation, policies, and procedures.
• Completing Mental Capacity Assessments and supporting individuals who lack capacity through Best Interest decision-making processes.
• Assessing and managing risks effectively, balancing individual choice with safety and safeguarding responsibilities.
• Working collaboratively with service users, carers, health professionals, housing providers, community services, and partner agencies to ensure coordinated support.
• Providing information, advice, and guidance regarding services available through statutory, voluntary, and community organisations.
• Supporting individuals who are at risk of hospitalisation or admission to long-term residential care by developing preventative and strengths-based interventions.
• Preparing high-quality assessments, reports, support plans, reviews, and professional documentation.
• Maintaining accurate, timely, and legally compliant records in accordance with organisational requirements.
• Supporting the development of students, care support workers, and colleagues through mentoring, coaching, and professional guidance.
• Contributing to service development, quality assurance activities, and continuous improvement initiatives across Adult Social Care.

Qualifications & Experience:

To be successful in this role you must have,

• Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, CSS, or equivalent recognised Social Work qualification.
• Registration: Must be registered with Social Work England.
• Experience: Significant post-qualified experience within Adult Social Care services.
• Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, and associated statutory guidance.
• Assessment Skills: Proven ability to undertake complex assessments, reviews, support planning, and risk management interventions.
• Safeguarding Experience: Experience undertaking safeguarding enquiries and implementing safeguarding protection plans.
• Care Management Experience: Experience coordinating care packages, monitoring services, and reviewing outcomes.
• Multi-Agency Working: Strong experience working collaboratively with health professionals, community services, housing teams, care providers, families, and partner agencies.
• Communication Skills: Excellent communication, report-writing, analytical, negotiation, and decision-making skills.
• IT Skills: Competent in the use of case management systems, electronic records, Microsoft Office, and related IT systems.
• Driving Licence: Full UK driving licence and willingness to travel across the service area.
• Professional Practice: Commitment to strengths-based, person-centred practice and continuous professional development.
