Job summary

Liquid Personnel is seeking an Adult Social Worker to deliver high-quality, person-centred social work within a multi-disciplinary rehabilitation team based in Bromley. The role focuses on timely assessments, care planning, and promoting recovery, independence, and safe discharge.

You will work with adults with neurological conditions, complete safeguarding, and contribute to service development. You must be registered with Social Work England and hold a UK driving licence.

Qualifications

• Social Work England registration.
• Eligible to work in the UK.
• Hold a full UK driving licence.
• Post-qualified experience.
• Degree level or equivalent in Social Work.

Responsibilities

• Undertake Care Act 2014 assessments and develop strengths-based support plans for neuro-rehabilitation or post-hospital discharge.
• Collaborate with occupational therapists, physiotherapists, nurses and discharge teams for safe transitions from acute settings.
• Complete Mental Capacity Assessments and Best Interest decisions; contribute to DoLS processes where required.
• Support individuals with acquired brain injuries, stroke, MS and other neurological conditions through rehabilitation and community reintegration.
• Respond to safeguarding concerns and contribute to Section 42 enquiries.
• Monitor and review care packages to meet evolving needs and promote independence.
• Maintain accurate case records using the council’s case management system (e.g., Liquid Logic, Mosaic).
• Advocate for service users and families; support voice in decision-making.
• Contribute to service development and continuous improvement within rehab and D2A pathways.

It focuses on timely assessments, care planning, and promoting recovery, independence, and safe discharge.

What will your responsibilities be?
