Are you a Newly Qualified, or an experienced Social Worker, registered with Social Work England, looking for your next challenge? Are you passionate about improving outcomes for adults and seeking a working environment that balances personal wellbeing with continuous improvement? Then Cheshire West and Chester Council could be the perfect place for you. We are seeking six new social workers to join our Hospital Discharge team, either on a permanent or temporary basis. The positions are based council wide and if successful you will be working at both hospitals and council offices.

These roles cover any Cheshire West resident needing discharge support to leave hospital. However, we work with our team to consider preferred locations to maximise their time and use of skills.You will work hand in hand with medics, therapy, nursing & reablement teams all focused with the same goals of completing personalised holistic assessment to support patients home safely and without delay. It is an opportunity to celebrate your social work value base and ensure that the voice of the individual and their carers are acknowledged and form the basis of any discharge plan.

As a Social Worker in the team, you would have the following responsibilities;

  • Be person centred and reinforce a patient's choice and control.
  • Promote a patient's well being throughout their journey in the acute and community setting.
  • Provide Strength Based Assessments and independence.
  • Complete Care Act 2014 compliant assessment.
  • Guide and support colleagues less experienced in social care processes.
  • Knowledge related legislation, policy, and frameworks.
  • Experience of Safeguarding processes and procedures.
  • Complete mental capacity assessment and best interest processes for patients unable to make specific decision.
  • Knowledge of the Deprivation of Liberty Safeguards.
  • Attend MDT's, professionals' meetings, discharge planning meetings and best interests' meetings as may be required.

We are looking for dedicated and highly motivated team players to join us as Social Workers working in line with the Care Act 2014. Our Social Workers play a critical role in supporting people to develop creative and innovative solutions to meet the outcomes they want to achieve.

You will have the opportunity to;

You will have:

  • CQSW/CSS/Diploma/Degree in Social Work or equivalent (Essential)
  • Social Work England Registration (Essential)
  • Be required to undertake an enhanced DBS plus Adults Barred List (Essential)
  • Statutory and policy base of Social Work (Essential)
  • Appreciation of recent developments in service provision.
  • Ability to communicate effectively with customers, carers and families.
  • Ability to cope positively with change and an ability to promote positive changes to make a difference.
  • Awareness of importance of multi-disciplinary working and an integrated approach.
  • Ability to carry out assessment and support planning in line with relevant legislation.
  • You are confident, energetic and enthusiastic as well as sensitive and resilient.
  • Self-motivating individual with the passion to make a difference in adult social care.
  • Resilient personal stress management.

This is a hybrid post which means working in one of our Council offices in locations across the borough as well as the opportunity to work from home where suitable.
