About The Role

Working Hours: 35 Hours Per Week

Contract Type: Permanent, Full-Time

Additional Payments: 6.7% Flexibility Payment, Essential Car User Allowance

Closing Date: 20 May 2025

This Role is Subject to:

  • Qualification: Social Work Qualification and registration with Social Work England
  • Medical Clearance: Pre-placement health assessment (PPHA)
  • Essential Car Documentation: This post is a designated Essential Car User. You must hold a full valid driving license and provide your own car. You will receive an Essential Car User Allowance.
  • DBS Clearance: This role requires an Enhanced Disclosure and Barring Service (DBS) check, in accordance with the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (amended 2013).

The Discharge to Assess Team is responsible for supporting timely and safe transfer of care from hospital beds when citizens are medically fit for discharge. The role involves working across hospital settings, Intermediate Care Units, and the Community, in partnership with health colleagues, to assess and plan transfers of care, promoting independence and a strength-based approach. The role requires independent working, evidence-based decision making, and accountability for complex social work duties.
