Job summary

Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s team, based in Conwy. The role covers hospital discharge planning, safeguarding, and complex caseload management within a Welsh context.

The successful candidate will hold Social Work England registration, be eligible to work in the UK, and possess a full UK driving licence, with post-qualification experience and a Social Work degree. This is a hybrid role offering supervision and training opportunities.

Responsibilities

• Hospital discharge planning— Lead safe and timely discharges by completing strengths‑based assessments, coordinating MDT input, and arranging appropriate community or residential support in line with the Social Services and Well‑being (Wales) Act 2014.
• Adult safeguarding— Identify, investigate, and manage safeguarding concerns for older adults and vulnerable adults, undertaking Section 126 enquiries, risk assessments, and protection planning with partner agencies.
• Complex case management — Hold a varied caseload including hospital‑based referrals, cognitive decline, physical frailty, and long‑term conditions; complete Care and Support Plans, Mental Capacity Act assessments, and best‑interest decisions.
