Job summary

Liquid Personnel is recruiting an Adult's Advanced Practitioner to join its client’s Community Social Work team, based in Nottinghamshire. This role offers professional leadership, supervision, and mentoring to social care staff while maintaining a reduced caseload of highly complex cases.

You will support adults across the community with assessments, care planning, safeguarding, and statutory interventions.

Job description

Liquid Personnel is recruiting an Adult's Advanced Practitioner r to join its client’s Community Social Work team, based in Nottinghamshire.

This is an excellent opportunity for a highly skilled social work professional to take a lead role in promoting high-quality, strengths-based practice whilst supporting the professional development of colleagues and delivering positive outcomes for adults across the community.

The successful candidate will provide professional leadership, supervision, mentoring, and support to social care staff whilst maintaining a reduced caseload of highly complex cases. Working with adults of all ages, you will undertake assessments, care and support planning, safeguarding, risk management, and statutory interventions, ensuring individuals are supported to maximise independence, wellbeing, choice, and control.

You will play a key role in embedding strengths-based, person-centred, and rights-based practice across the team, promoting innovative approaches that enable people to manage their own needs, risks, and uncertainties whilst delaying or preventing the need for long-term care and support.

What will your responsibilities be?

  • Acting as a lead practitioner within the team, promoting high-quality, evidence-based social work practice and providing professional leadership to colleagues.
  • Managing a reduced caseload of highly complex cases, undertaking assessments, interventions, reviews, and support planning.
  • Completing strengths-based Care Act Assessments and identifying eligible needs, outcomes, strengths, and community-based solutions.
  • Supporting the development of best practice across key areas including safeguarding, risk enablement, Mental Capacity Act, Mental Health Act, and person-centred practice.
  • Undertaking Mental Capacity Assessments, Best Interests decisions, and applying relevant legislation to support decision-making and safeguard vulnerable adults.
  • Undertaking the role of Approved Mental Health Professional (AMHP) or Best Interests Assessor (BIA) where appropriate to the service area.
  • Providing regular professional supervision, coaching, mentoring, and reflective practice sessions to support staff development and promote effective social work practice.
  • Supporting workforce development by identifying learning needs and contributing to training, professional development, and service improvement initiatives.
  • Leading on safeguarding concerns and complex risk management situations, ensuring appropriate interventions are implemented to protect vulnerable adults.
  • Developing and reviewing person-centred care and support plans that promote independence, wellbeing, and community inclusion.
  • Supporting effective allocation and management of cases across the team, ensuring resources are used effectively to meet service demands.
  • Chairing meetings, case discussions, and professional forums, providing expert advice and guidance to practitioners and partner agencies.
  • Working collaboratively with individuals, carers, families, health professionals, providers, and partner organisations to deliver coordinated and effective support.
  • Preparing and presenting high-quality assessments, reports, case summaries, and professional recommendations.
  • Supporting service performance and continuous improvement initiatives, identifying issues that may impact performance, quality, or budget requirements.
  • Contributing to partnership development and maintaining effective working relationships with internal and external stakeholders.
  • Ensuring statutory duties and organisational responsibilities are delivered in accordance with legislation, policy, and best practice guidance.

Qualifications and Experience:

To be successful in this role you must have;

  • Professional Qualification:MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.
  • Registration:Must be registered with Social Work England.
  • Experience:Significant post-qualified experience within Adult Social Care, including experience managing complex cases and supporting professional practice.
  • Leadership Experience:Experience providing supervision, mentoring, coaching, or professional leadership within a social work setting.
  • Legislation Knowledge:Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983, Human Rights Act 1998, Adult Safeguarding legislation, and associated statutory guidance.
  • Assessment Skills:Proven ability to undertake complex assessments, risk management, support planning, and statutory social work interventions.
  • Safeguarding Experience:Extensive experience managing safeguarding concerns and supporting best practice in safeguarding adults.
  • AMHP/BIA Qualification:AMHP or BIA qualification is desirable, depending on service requirements.
  • Strengths-Based Practice:Demonstrable experience embedding strengths-based, person-centred, and rights-based approaches within Adult Social Care.
  • Multi-Agency Working:Ability to establish and maintain effective relationships with health services, providers, carers, families, and partner agencies.
  • Communication Skills:Excellent communication, report-writing, analytical, leadership, and decision-making skills.
  • Performance Management:Experience supporting service improvement, workforce development, and quality assurance activities.
  • Driving Licence: Full UK driving licence and access to a vehicle is desirable due to the community-based nature of the role.
