Job summary

Liquid Personnel is seeking an experienced Adult's Social Worker to join a Community team based in the Bournemouth, Christchurch and Poole area. You will support adults with long-term conditions, undertake Care Act assessments, safeguarding, and care planning to promote independence.

The role requires extensive assessment skills, safeguarding experience, and Court of Protection involvement, with a commitment to strengths‑based, person‑centred practice working with families, carers, and health

Qualifications

  • Professional qualification in social work (MA/BA/BSc) or equivalent.
  • Registration with Social Work England is mandatory.
  • Experience in Adult Social Care and community practice.
  • Strong understanding of Care Act 2014 and Mental Capacity Act 2005.

Responsibilities

  • Manage a caseload of adults with long-term health conditions in a community setting.
  • Undertake Care Act 2014 assessments, reviews, reassessments and support planning.
  • Develop and review person-centred Care and Support Plans to promote independence.
  • Arrange and coordinate care packages and community services.
  • Conduct safeguarding enquiries in accordance with safeguarding policies.
  • Manage complex cases including self-neglect and risk management.
  • Undertake Mental Capacity Assessments and Best Interests decisions.
  • Carry out Court of Protection work and prepare professional reports when required.
  • Collaborate with health professionals, care providers, and families to achieve positive outcomes.
  • Maintain accurate records and participate in multi-disciplinary meetings.

Job description

Liquid Personnel is recruiting an Adult's Social Worker to join its client's Community team, based in Bournemouth, Christchurch and Poole. This is an excellent opportunity for a dedicated practitioner to support adults with a range of long-term conditions and complex care and support needs, promoting independence, wellbeing, and positive outcomes through strengths-based and person-centred social work practice.

The successful candidate will manage a varied community-based caseload, undertaking statutory assessments, reviews, safeguarding interventions, and support planning. Working closely with service users, carers, families, health professionals, and partner agencies, you will play a key role in ensuring individuals receive appropriate support to remain safe, independent, and connected to their local communities.

This role requires a confident and experienced Social Worker with strong assessment skills, safeguarding knowledge, and experience of Court of Protection work, alongside the ability to manage complex casework involving self-neglect, risk management, and long-term health conditions.

What will your responsibilities be?
  • Managing a caseload of adults with long-term health conditions and complex care and support needs within a community setting.
  • Undertaking Care Act 2014 assessments, reviews, reassessments, and support planning to identify eligible needs and achieve positive outcomes.
  • Developing, implementing, and reviewing person-centred Care and Support Plans that promote independence, wellbeing, and choice.
  • Arranging and coordinating care packages, community support services, and interventions to meet assessed needs.
  • Undertaking safeguarding enquiries and interventions in accordance with Adult Safeguarding legislation, policies, and procedures.
  • Managing cases involving self-neglect and working proactively with individuals to reduce risk and improve wellbeing.
  • Completing Mental Capacity Assessments and contributing to Best Interest decision‑making in accordance with the Mental Capacity Act 2005.
  • Assessing and managing complex risks, balancing safety, choice, control, and positive risk‑taking.
  • Undertaking Court of Protection work, including preparing professional reports, assessments, and supporting legal processes where required.
  • Working collaboratively with health professionals, community services, care providers, carers, families, and partner agencies to achieve positive outcomes.
  • Participating in multidisciplinary meetings, safeguarding discussions, reviews, and case conferences as required.
  • Maintaining accurate, timely, and legally compliant records, assessments, reports, and support plans.
  • Promoting strengths‑based practice and supporting individuals to access community resources, informal networks, and preventative services.
Qualifications and Experience:

To be successful in this role you must have;

  • Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, CSS, or equivalent recognised Social Work qualification.
  • Registration: Must be registered with Social Work England.
  • Experience: Significant post-qualified experience within Adult Social Care services.
  • Community Experience: Experience working with adults with long-term conditions and complex needs within a community‑based Adult Social Care setting.
  • Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, Court of Protection processes, and associated statutory guidance.
  • Assessment Skills: Proven ability to undertake Care Act assessments, reviews, support planning, and risk management interventions.
  • Safeguarding Experience: Experience undertaking safeguarding enquiries and managing complex safeguarding concerns.
  • Self‑Neglect Experience: Experience working with adults who self‑neglect and implementing effective intervention and risk management plans.
  • Court of Protection Experience: Experience preparing reports and supporting Court of Protection applications and proceedings.
  • Multi‑Agency Working: Strong experience working collaboratively with health professionals, community services, care providers, families, and partner agencies.
  • Communication Skills: Excellent communication, report‑writing, analytical, negotiation, and decision‑making skills.
  • Professional Practice: Commitment to strengths‑based, person‑centred practice and continuous professional development.
  • Organisational Skills: Ability to work independently, manage complex caseloads, and prioritise effectively within a demanding community service.
