Qualifications

  • Hold a recognised Social Work qualification and be registered with Social Work England.
  • Sound understanding of Care Act 2014, Mental Capacity Act 2005 and safeguarding guidance.
  • Commitment to high-quality, person-centred practice.
  • Excellent communication, organisational and IT skills.
  • Ability to manage competing priorities in a busy environment.
  • Demonstrate resilience, curiosity and reflective practice.
  • Ability to work independently while contributing to the team.

Responsibilities

  • Undertaking strengths-based assessments and reviews.
  • Developing person-centred care and support plans.
  • Working collaboratively with families, carers, health colleagues, providers and partner agencies.
  • Supporting safeguarding activity and risk management.
  • Contributing to Continuing Healthcare assessments and attending Decision Support Tool meetings.
  • Promoting independence through community resources and informal networks.

About The Role

You will work with adults who require support to maintain their independence, improve their wellbeing and achieve their personal outcomes.

Key responsibilities include:

About You
  • Hold a recognised Social Work qualification and be registered with Social Work England.
