Job summary

Tameside Council is seeking qualified Social Workers to join Adult Services in Hyde. The role focuses on enabling adults to live well at home, managing Care Act Assessments, Support Plans, and Best Interest Decisions.

You will join a supportive Neighbourhood team, with extensive training, supervision, and opportunities for progression from ASYE to senior leadership. Flexible working and staff development are prioritised at Tameside.

Qualifications

• Qualified Social Worker with ASYE completed.
• Experience managing caseloads in adult social care.
• Strong communication and relationship-building with health and social care colleagues.

Responsibilities

• Lead on supporting adults to live well at home and manage a caseload.
• Conduct Care Act Assessments, Support Plans and Mental Capacity Assessments.
• Make Best Interest Decisions and signpost to internal and external services as needed.
• Provide professional guidance and signposting to promote wellbeing and independence.
• Engage with multidisciplinary teams to coordinate responses for individuals and families.
• Participate in safeguarding and continuous service improvement initiatives.

Salary: Grade H. Scale 29 - £36,601 to Scale 34 - £41,403 with a professional bar at Scale 31 - £38,354** (actual salary)

About You

You will take the lead on supporting adults to live well at home. You will be part of a friendly and supportive team and your work will include managing a caseload, Care Act Assessments, Support Plans, Mental Capacity Assessments and Best Interest Decisions and signposting to internal and external services where necessary to enhance wellbeing and independence.

You will have excellent communication skills and have the ability to build effective professional relationships with other disciplines and with other local health and social care services to enable a coordinated response to individuals and families.

** This post is classified as being in a professional job category. As such the grade for the job has a professional grade development bar which employees can progress through subject to meeting the following criteria:
