Job summary

Gloucestershire County Council seeks a Social Worker to join its new Community Reablement Service in Gloucestershire. You will undertake Strengths-based Care Act assessments for adults needing ongoing care and support after reablement and develop person-centred support plans to promote independence and wellbeing.

You will work within an integrated multidisciplinary team, collaborate with OT, Social Care Practitioners and Reablement Providers, and lead safeguarding enquiries as required.

Qualifications

  • Registered Social Worker with experience in Adult Social Care.
  • Experience of Care Act assessments, support planning and adult safeguarding.
  • Sound understanding of relevant legislation including Care Act 2014 and Mental Capacity Act 2005.

Responsibilities

  • Applying the Mental Capacity Act 2005, including capacity assessments and best-interest decision-making.

Our new Community Reablement Service is seeking a passionate, skilled and motivated Social Worker to join our countywide team. This is a unique opportunity to be part of an innovative service from the very beginning, helping to build and influence how we deliver support to the people of Gloucestershire.

Working as part of an integrated multidisciplinary team, you will play a key role in supporting adults at a critical stage in their journey. Where it appears an individual may require ongoing care and support beyond their reablement period, you will undertake strengths-based Care Act assessments, identify eligible needs, and develop person-centred support plans that promote independence, wellbeing and choice.

As a registered Social Worker, you will also hold responsibility for safeguarding enquiries and safeguarding practice in line with the Care Act 2014, ensuring adults at risk are supported, protected and empowered to achieve the outcomes that matter most to them.

The service brings together professionals from a range of disciplines, working as one team to support people to achieve what matters most to them. We focus on enabling people to do as much as possible for themselves, promoting strengths, resilience and community connections.

About the Role

As a Social Worker within our new Community Reablement Service, you will undertake strengths-based Care Act assessments for adults who may require ongoing care and support following reablement.

Working closely with individuals, carers and families, you will develop person-centred support plans that promote independence, wellbeing and choice. Working as part of an integrated multidisciplinary team, you will collaborate with Occupational Therapists, Social Care Practitioners, Reablement Assessors, Community Reablement Providers and wider Adult Social Care colleagues to ensure seamless transitions and coordinated support.

You will use your professional expertise to manage complex situations, undertake risk assessments, apply the Mental Capacity Act 2005, and lead safeguarding enquiries in line with the Care Act 2014. You will also provide professional guidance to colleagues and contribute to the ongoing development of this innovative service. Key responsibilities include:

  • Undertaking Care Act assessments and support planning.

The role is based at Shire Hall, Gloucester, with travel across Gloucestershire as part of our countywide service delivery model.

About You

We are looking for a registered Social Worker with experience in Adult Social Care who are committed to delivering strengths-based, person-centred practice. You will have experience of Care Act assessments, support planning and adult safeguarding, alongside a sound understanding of relevant legislation including the Care Act 2014 and Mental Capacity Act 2005.
