Job summary

Gloucestershire County Council seeks a Social Worker to join its new Community Reablement Service in Gloucestershire. You will undertake Strengths-based Care Act assessments for adults needing ongoing care and support after reablement and develop person-centred support plans to promote independence and wellbeing.

You will work within an integrated multidisciplinary team, collaborate with OT, Social Care Practitioners and Reablement Providers, and lead safeguarding enquiries as required.

Qualifications

• Registered Social Worker with experience in Adult Social Care.
• Experience of Care Act assessments, support planning and adult safeguarding.
• Sound understanding of relevant legislation including Care Act 2014 and Mental Capacity Act 2005.

Responsibilities

• Undertaking Care Act assessments and support planning.
• Applying the Mental Capacity Act 2005, including capacity assessments and best-interest decision-making.
• Leading and contributing to safeguarding enquiries and risk management.
• Working collaboratively to deliver coordinated, person-centred support.
• Promoting access to community resources, local services and informal support networks.
• Supporting service development through innovation and continuous improvement.
