Job summary

NHS Highland is seeking a skilled Social Work Assistant Practitioner to join the Adult Services Social Work Team. The role involves supporting adults, families and carers in a statutory setting, contributing to assessment, care planning and service coordination.

You will work with Social Workers and colleagues to support discharge planning and community-based interventions, with emphasis on care, compassion, dignity and respect and opportunities for learning and career progression.

Qualifications

• Experience in Adult Social Care is required.
• Qualified to SVQ Level 3 or above, or hold an HNC in Health and Social Care (or equivalent).
• Committed to person-centered, strengths-based support.

Responsibilities

• Support adults, families and carers within a statutory social work setting.
• Contribute to assessment, care planning and service coordination.
• Assist with hospital discharge planning and community-based interventions.
