Job summary

You will work closely with Social Workers and multidisciplinary colleagues, including supporting hospital discharge planning and communitybased interventions.

You must have experience in Adult Social Care and be qualified to SVQ Level 3 or above, or hold an HNC in Health and Social Care (or equivalent).
