About the role

Are you passionate about supporting older adults to live safely, independently, and with dignity?

We are looking for committed Social Work Assistant to join our dedicated Older People’s Community Team. In this role, you’ll assess the needs of people aged 65+ and develop personalised care plans that promote independence, wellbeing and quality of life. You’ll work closely with individuals, families and carers, using strength‑based approaches to support people to stay connected to their communities.

You’ll work collaboratively across a wide range of services, including hospitals, community teams and partner organisations, helping ensure smooth and effective support for those in your care. Working as part of a supportive and dedicated team, you’ll play a key role in improving outcomes for older people across the Island.
