Job summary

Bradford District Care NHS Foundation Trust is seeking a motivated Tobacco Dependency Advisor to join our Community Mental Health Teams. You will deliver evidence-based tobacco dependency treatment and behavioural support to help people reduce or stop smoking within mental health services.

Working as part of a multidisciplinary team, you will develop personalised care plans, support access to nicotine replacement therapy, and promote healthy lifestyles to reduce health inequalities among people

Qualifications

• GCSE English and Maths at minimum level C.
• NVQ level 3 in Health and Social Care (or equivalent).
• Smoking Cessation Training Level 2 or willingness to undertake.
• Training in health promotion interventions.
• Experience or training in delivering behavioural change / motivational interventions.

Responsibilities

• Deliver one-to-one tobacco dependency assessments and behavioural support.
• Develop personalised care plans and contribute to multidisciplinary care planning.
• Support access to nicotine replacement therapy, stop smoking medications and harm reduction interventions.
• Provide advice on tobacco dependency, vaping and smoking cessation options.
• Promote healthy lifestyles and use Making Every Contact Count approach; ensure continuity of care between services.
• Monitor outcomes, maintain accurate records and support audit and evaluation.
