Job summary

An exciting opportunity has arisen for a Sister/Charge Nurse/District Nurse to join our Primary Care Home team, delivering high-quality nursing care and providing care at home to patients registered with GP surgeries in Waveney. The PCH team collaborates closely with GPs and other multidisciplinary team members to provide person-centered nursing care to adult patients in their homes, residential care, or clinics, supporting self-management whenever possible. The service utilizes SystmOne as a clinical IT system to support mobile working.

The successful candidate will manage a caseload, ensuring high standards of evidence-based nursing care. They should demonstrate leadership, organizational, and communication skills, with the ability to influence and manage change.

District Nursing is a challenging, varied, and rewarding field; the service needs to be flexible to meet patient needs. Main duties of the job

The post is full-time, within a multidisciplinary team providing community services 7 days a week from 07:00 to 20:30.

You must be a registered nurse with at least 2 years of community experience and a desire to develop further.

The role offers variety daily, including clinical practice, triage, leadership, and management. Responsibilities include line managing several clinicians, participating in HR processes such as recruitment and appraisals, and supporting junior staff.

The main duties include:

  • Providing clinical leadership, operational management, and professional support within the Primary Care Home.
  • Collaborating with the multidisciplinary team for expert case management of community patients requiring planned and unplanned nursing care.
  • Working autonomously within your clinical competence and scope of practice.
  • Carrying out rapid assessments in patients' homes to prevent hospital admissions, considering physical, physiological, and social needs.
  • Being an autonomous practitioner responsible for your caseload and leading service improvement projects.
  • Contributing to service evaluation to ensure it remains fit for purpose.

  • Manage medication prescribing and review within scope of practice, ensuring safe use of equipment and resources.
  • Contribute to staff training, clinical audit, risk management, and resource efficiency.

Person Specification
Qualifications
  • Registered Nurse with NMC (Adult) registration.
  • BSc in Nursing Practice or equivalent.
  • Evidence of ongoing professional development.
  • Leadership/management qualification or willingness to undertake.
  • Health Coaching qualification.
  • Specialist Practitioner in District Nursing.
  • Community Nurse Prescriber (V100/V150).
  • Independent non-medical prescriber.
Skills and Knowledge
  • Sensitivity in handling anxious patients.
  • Strong assessment and patient management skills.
  • Understanding of legal and ethical issues in nursing.
  • Excellent communication, negotiation, and organizational skills.
