An exciting opportunity has arisen for a Sister/Charge Nurse/District Nurse to join our Primary Care Home team, delivering high-quality nursing care and providing care at home to patients registered with GP surgeries in Waveney. The PCH team collaborates closely with GPs and other multidisciplinary team members to provide person-centered nursing care to adult patients in their homes, residential care, or clinics, supporting self-management whenever possible. The service utilizes SystmOne as a clinical IT system to support mobile working.
The successful candidate will manage a caseload, ensuring high standards of evidence-based nursing care. They should demonstrate leadership, organizational, and communication skills, with the ability to influence and manage change.
District Nursing is a challenging, varied, and rewarding field; the service needs to be flexible to meet patient needs. The post is full-time, within a multidisciplinary team providing community services 7 days a week from 07:00 to 20:30.
You must be a registered nurse with at least 2 years of community experience and a desire to develop further.
The role offers variety daily, including clinical practice, triage, leadership, and management. Responsibilities include line managing several clinicians, participating in HR processes such as recruitment and appraisals, and supporting junior staff.
