We have an opportunity for senior nurses to join our Adult Community Team, based out of Fair Havens hospice, located near beautiful Priory Park. This service runs across 7 days, and there is a requirement for evening and weekend working.
You will support and manage our community team members during a shift to enable patients' care needs to be met, being responsible for assessing the needs of patients, implementing and evaluating the care plans provided.

Our specialist Care Teams can support them and their family living throughout illness, death, and bereavement in the comfort of their own home and through our hospice services, Fair Havens and Little Havens.
