Community Integrated Care is recruiting for a Service Leader Level 3 in Manchester to provide person-centred support to older adults in Extra Care housing. You will help residents live independently with personalised support, meals, personal care and daily tasks, while promoting dignity and wellbeing.
The role requires a professional qualification in Health and Social Care and experience in health or social care settings.

This is an Extra Care Service supporting older adults aged 65 and over to live independently in their own apartments, whilst having access to personalised support when they need it. We current support 16 individuals and our approach is centred around promoting choice, independence, dignity and wellbeing, ensuring every person is supported in a way that reflects what matters most to them.
We provide medical support, meal preparation, personal care, moving and positioning, household tasks and cleaning and shopping and accessing the local community.
Monday to Friday 37.5 hour general office hours but flexibility is required for night checks and other needs.
With experience working in a health or social care setting, you will have an upbeat positive manner, as well as having a genuine passion for supporting people to become a part of their local and wider community.
You will also have experience working with people supported who present behaviours that require intensive support planning and management to ensure their safety and quality of life at all times.
In addition you will be comfortable using basic IT systems that are required within the role - training can be offered to support this area. You will also hold a professional qualification in Health and Social Care, i.e. Nurse, Social Worker, SVQ Level 3 plus supervisory units (Scotland) / Care Management Level 3 (England).
